The story of one transformation

Complaints:

The patient is a 27-year-old woman. Since the age of 14, she had been using a removable partial denture that inhibited the growth of the upper jaw. This led to severe atrophy of the alveolar ridge and pronounced dentoalveolar deformities.

Solution:

Comprehensive surgical and prosthetic treatment was carried out. The roots of the teeth in both the upper and lower jaws were removed. Reconstruction of the alveolar process of the upper jaw was performed, including bilateral open sinus lift, vertical bone augmentation, and reinforcement of the graft with a titanium mesh based on an individually 3D-printed model with virtual planning. Horizontal augmentation of the alveolar ridge of the lower jaw in edentulous areas was also performed. Subsequently, the titanium mesh was removed, tooth 1.6 was extracted, and 9 implants were placed. The final stage included placement of gingival formers and soft tissue plastic surgery of the oral vestibule.

Several days of severe toothache that could not be ignored

Complaints:

The patient had been suffering from severe toothache for several days. In addition, she was using removable dentures that were poorly fixed and did not provide comfort or aesthetics. We first resolved the urgent condition, and this is when trust was established between us. After the first CT scan, it became clear that the optimal solution would be a full-arch surgery using the All-on-4 protocol with immediate loading. At the follow-up visit, we agreed on this treatment plan.

Solution:

Preliminary diagnostics and virtual planning were carried out. According to the All-on-4 protocol, under medical sedation, 8 Nobel Biocare Parallel CC implants were placed. On the same day, the patient received temporary prostheses, and after 10 days — permanent ones. Thanks to the guided protocol and precise planning, healing was fast and almost painless. Due to the specifics of this design, defects in the bone tissue developed over time, and both use and maintenance became significantly more complicated. All-on-4 is a full-arch restoration technique in which a fixed prosthesis is supported by just four implants. This approach allows restoration of chewing function, comfort, and smile aesthetics even in cases of complete or near-complete tooth loss. It is suitable for patients who have lost most or all of their teeth, do not want removable dentures, and are looking for a reliable, functional solution. In many cases, All-on-4 also helps avoid complex additional procedures and allows a faster return to normal life.

What is the difference between All-on-4 and All-on-6?

The difference lies in the number of implants (4 or 6), which affects the stability of the structure and the distribution of chewing load. All-on-6 provides greater stability, while All-on-4 is often suitable in cases with limited bone volume.

Is it possible to get implants if a tooth was lost long ago?

Yes, implantation can be performed even long after tooth loss, but the doctor will first assess the condition of the bone tissue, and if necessary, bone augmentation may be required.
